Tier II Strong human biomarker data; clear mechanism of action; active Phase II/III trials.
Tier III Observational human data or strong primate data; conflicting or early RCTs.
Tier IV Murine/in-vitro only. Unproven in humans. (Not listed).

Tier II mTOR Inhibitor

Rapamycin (Sirolimus)

FDA approved immunosuppressant currently undergoing trials for geroprotection via mTORC1 inhibition.

Typical Off-Label Dosing

5-8mg orally, once weekly.

Pulsed dosing used to avoid mTORC2 inhibition and mitigate immunosuppression.

Tier III Biguanide

Metformin

First-line Type 2 Diabetes medication with observed geroprotective effects in retrospective epidemiological studies.

Typical Off-Label Dosing

500mg - 1000mg extended release, daily.

  • AMPK activation
Recent Update: Recent ITP (Interventions Testing Program) data showed negligible lifespan extension in mice when started in middle age. TAME (Targeting Aging with Metformin) trial pending funding. Evidence downgraded from Tier II to Tier III.
